Job Description:

Join a rewarding travel healthcare opportunity as a Case Manager Registered Nurse at a facility in San Leandro, California. This role involves managing patient discharge plans, assessing needs, and coordinating with the care team within the Acute Care and Med/Surg settings. You'll utilize your expertise with EMR systems like EPIC, and your experience with admission criteria, disease management, and compliance standards such as CMS, HIPAA, OSHA, and Joint Commission is essential. This assignment is designed to challenge your clinical skills while offering a chance to experience a different healthcare environment. Occasionally, there may be a need to swap to Med/Surg call or float, providing variety in your daily responsibilities. The shift is a consistent 8-hour day, five days a week, for a total of 13 weeks.
