Job Description:

Seeking a dedicated discharge planner to join a 13-week contract in Fort Defiance, AZ. This role involves coordinating discharge planning for patients with chronic diseases and special healthcare needs within various inpatient units, including ICU, OB, and Adolescent Care. The position offers an engaging opportunity to work closely with a multidisciplinary team, ensuring smooth transitions for patients returning home or transferring to other facilities. Expect a Monday through Friday schedule, with 8-hour days focused on patient chart reviews, tracking system management, and referral coordination. This is a chance to leverage your RN experience in a fast-paced, hospital setting while making a tangible difference in patient outcomes.
