Job Description:
Join a 13-week travel healthcare assignment as a Care Manager supporting patient progression from admission through discharge. In this fast-paced acute hospital role, you will coordinate care, utilization review, discharge planning, and transitions across the continuum while partnering with patients, families, physicians, multidisciplinary teams, payors, and community resources. You will manage approximately 20–25 cases each day, use MCG and InterQual criteria, document in EPIC, identify barriers to discharge, and help develop safe, appropriate plans for each patient. The schedule is five 8-hour days, with occasional 7:30 a.m. arrival and flexibility to remain until daily cases are complete.Weather:
