Job Description:

Join us as a Case Manager Registered Nurse in Oakland for a 13-week contract, where you'll play a vital role in ensuring smooth care transitions and discharge planning within the hospital setting. This position involves collaborating closely with physicians, social workers, and the interdisciplinary team to coordinate patient care, optimize resource utilization, and facilitate timely transfers from inpatient to post-acute care. Job Specifics:

Location: Oakland, California
Shift: 8-hour days, 5 days a week (08:00 - 16:30), every other weekend off
