Job Description:

This travel contract is designed for an RN or LSW Case Manager supporting adults in an inpatient acute rehabilitation setting. You will coordinate services, assess needs, support discharge planning, and help patients and families navigate the next steps following rehabilitation for conditions such as stroke, spinal cord injury, orthopedic recovery, and medically complex needs. The assignment offers weekday day shifts, no weekend or call requirements, and the opportunity to work as part of a rehabilitation-focused team.
