Job Description:

Join a 13-week travel healthcare assignment as an RN Case Manager in Portland, Maine. In this weekday care management role, you will support discharge planning and care coordination for high-acuity adult patients, including a significant behavioral health population. You will collaborate across the care management department, help patients transition safely through the healthcare system, and use EPIC and related care coordination platforms to organize each plan. The schedule includes eight-hour day shifts, Monday through Friday, with weekend and holiday coverage requirements. A calm, flexible approach will be valuable as you solve complex discharge needs and help keep every transition moving forward.
