Job Description:

Support acute care patients through safe, appropriate discharge planning as a Care Transitions Registered Nurse in Lynchburg, Virginia. You will assess clinical needs, coordinate transition plans, help ensure compliance with state and federal regulations, and collaborate with multiple disciplines to arrange the right level of post-acute care. The primary schedule is Monday through Friday during the day, with possible weekend and holiday coverage. The assignment may also include coverage in Bedford or Southside, including Farmville.
