Job Description:

Support patients and families through each stage of their care journey as a travel RN Case Manager in Hermiston, Oregon. Working 7:30 a.m. to 4:00 p.m., including weekends, you will assess healthcare needs, develop and monitor individualized care plans, evaluate medical necessity, coordinate transitions between care settings, and identify barriers to discharge. You will collaborate with physicians, therapists, social workers, insurers, community agencies, and post-hospital providers while advocating for patient preferences and access to appropriate resources. The assignment includes patient and family education, authorization coordination, thorough documentation, interdisciplinary care conferences, and connecting individuals with specialized services and community support.
