Job Description:

Join a wound care case management team where you'll take charge of assessing and supporting patients with complex wound healing needs. Your day will involve performing comprehensive wound assessments, documenting findings meticulously, and collaborating closely with physicians to develop and adapt personalized care plans. You'll assist with wound therapy protocols, coordinate patient education, and act as a vital link between the clinic, the patient's home, and other care providers. This role offers a hands-on opportunity to make a tangible difference in patients’ recovery journeys while managing a dedicated caseload in a fast-paced outpatient setting. Expect to engage in both clinical procedures and care coordination, ensuring seamless continuity and progress towards healing goals.
