Job Description:

Join the team at Johns Hopkins Hospital as an Adult Nurse Case Manager on a 13-week contract. This role offers the opportunity to coordinate patient transitions within a bustling hospital setting, focusing on adult medicine and surgery units. The position involves managing a caseload of patients from admission through safe transfer to home or post-acute facilities, ensuring smooth communication between payors, families, and the multi-disciplinary team. You'll use your inpatient case management skills, navigate clinical and fiscal data, and work within EPIC and other referral platforms. The schedule features consistent 8-hour daytime shifts—no nights, weekends, or holidays—allowing you to focus on delivering impactful patient care coordination and safety.
