Job Description:

Join a dedicated team on the acute rehabilitation unit at Charlton Memorial Hospital in Fall River. This role offers the chance to coordinate patient recovery and rehabilitation plans, supporting patients through their journey toward improved independence. You'll be working within a 30-bed facility, handling case management responsibilities with a focus on rehabilitation preferences. The position involves a Monday through Friday schedule, with standard 8-hour shifts.
