Job Description:

This home health therapy role offers a rewarding opportunity to deliver outpatient rehabilitation services in a community setting across multiple counties. You will work with patients in their homes, providing skilled physical therapy to improve mobility, strength, and functional independence. The schedule consists of five 8-hour days, with options for 4x10s if preferred, and a rotation that includes one Sunday every five weeks. During weekends assigned as admit days, you'll work Saturday and Sunday with two weekdays off, allowing for consistent routines. Experience in home health settings and a valid state license are required, along with the ability to work independently and adapt to varying patient needs. Prepare to engage in meaningful rehab activities that help patients regain their independence in familiar environments.
