Job Description:

This RN case management position places you in the heart of a short-term acute care facility known for its magnet designation and teaching status. You will engage in adult inpatient, emergency department, and pediatric case management, ensuring seamless patient transitions and care coordination. The role involves working five 8-hour days on a consistent day shift (7:00 am – 3:30 pm) for a duration of 13 weeks, with a guaranteed 40 hours per week. The facility values experienced professionals with at least two years of recent hospital case management, ideally with some travel background. While Epic charting experience isn't mandatory, it is highly preferred. The environment is fast-paced but rewarding, offering a chance to make a meaningful difference in patient outcomes while navigating a collaborative team setting. Flexibility within scope is available to meet facility needs, including possible float assignments across adult inpatient, ED, and pediatric units.
