Job Description:

Join a dedicated team providing outpatient physical therapy and rehabilitation services in a home health setting across a diverse region. This role involves delivering therapeutic interventions to patients in their homes, coordinating care during 10-hour shifts that may be scheduled as five 8-hour days or four 10-hour days, based on traveler preference. The position requires active state licensure, a minimum of one year of home health experience, and BLS certification. You’ll be the key to helping patients regain mobility and independence, working in various counties and occasionally managing admit days on weekends.
