Job Description:

This travel opportunity involves serving as an RN Care Coordinator (Case Manager) in a fast-paced healthcare setting. You will oversee a caseload of patients, coordinating care plans prescribed by an interdisciplinary team. Your role requires assessing patients from pre-admission through post-discharge, ensuring compliance with care guidelines, protocols, and payer requirements. This position offers the chance to apply your expertise in managing patient episodes of illness while supporting efforts to improve quality, efficiency, and patient satisfaction. Expect to navigate a variety of clinical scenarios that keep your skills sharp and your experience meaningful, all while maintaining a schedule of eight-hour shifts for 13 weeks.
