Job Description

Join a travel healthcare contract as a Case Manager Registered Nurse supporting an academic acute-care setting in St. Louis. In this Monday-through-Friday role, you will complete assessments for admitted patients, participate in interdisciplinary rounds, coordinate transition planning, and initiate appropriate referrals for home health, durable medical equipment, rehabilitation, and other post-acute services. Experience with EPIC and Allscripts is preferred. This assignment is designed for an experienced case management RN, with candidates bringing at least two years of case management experience in a comparable facility encouraged to apply.
