Job Description:

Join our team for a 13-week travel healthcare contract in Indianapolis, where you'll coordinate the comprehensive interdisciplinary plan of care for patients from admission through discharge. This role involves developing personalized care plans that incorporate patient and family preferences, as well as monitoring medical necessity and care levels through assessments and evaluations. You'll advocate for necessary resources and help remove barriers to ensure seamless transitions. Your day will include maintaining clear communication with supervisors and care transition teams to keep the patient’s health plan on track. Additionally, you'll serve as a resource for accurate medical record documentation, ensuring that level of care recommendations and services align with diagnoses and post-discharge needs.
