Job Description:

Work as a Registered Nurse Case Manager on a travel contract in Atlanta, Georgia, focused on driving the discharge plan forward with daily oversight and coordination. In this role, you will assess, evaluate, and help align services to the appropriate level of care, ensuring the treatment plan is initiated, reviewed, and re-evaluated each day. You will monitor progress and identify barriers to discharge while tracking and supporting length-of-stay goals.

You’ll also ensure compliance with Medicare-related requirements, including patient choice processes and IMM notices and MOONS. The schedule is Monday through Friday with no weekend shifts, call, or floating expected beyond facility needs within scope of practice.

This assignment is best suited for an experienced RN Case Manager (minimum 3 years), not a first-time traveler, and candidates will be reviewed through clinical screening to confirm fit before final manager approval.

Shift: 8 hour days (08:00 - 16:30), Monday–Friday; no weekends

Duration: 12 weeks
