Job Description:

Join a seven-week contract at UF Health Shands Hospital in Gainesville as a Registered Nurse – Case Manager. You will evaluate and coordinate discharge planning, ensuring seamless transitions for adult and pediatric patients in Med/Surg, ICU, Trauma, and Oncology units. Your expertise in patient assessments, collaboration with interdisciplinary teams, and knowledge of payer and regulatory standards will be vital in optimizing patient outcomes. This role offers the chance to work in a large, academic hospital setting with a focus on comprehensive case management across various acute care units. Expect to navigate complex care plans while contributing to a multidisciplinary team dedicated to patient progression and resource utilization.
