Job Description:

Join a travel contract as a Registered Nurse Case Manager in Atlanta, supporting daily discharge planning by assessing needs, evaluating progress, and driving the discharge plan forward. In this Monday–Friday role (weekends off), you will coordinate services to ensure the plan matches the appropriate level of care and that the treatment plan is initiated, evaluated, and re-evaluated each day. You’ll monitor length of stay, identify barriers to discharge, and follow Medicare regulatory guidelines related to patient choice and required notices (including IMM and MOONS). This assignment is designed for experienced case managers who can track progress daily and keep the discharge workflow moving with consistency and accuracy.

Weather:

Expect generally mild days with warm afternoons, occasional humidity, and the possibility of passing showers. Evenings can cool off, making for comfortable outdoor time between shifts.
