Job Description:

This 13-week contract is designed for experienced RN Case Managers ready to take on a pivotal role in inpatient acute care settings. You will coordinate patient care plans, facilitate smooth transitions between levels of care, and collaborate with multidisciplinary teams to prevent unnecessary readmissions. This position involves working 8-hour daytime shifts, with every other weekend off, offering a balanced work-life rhythm. Your expertise in hospital case management, along with strong communication skills, will be essential in navigating complex clinical and social situations.
