This case management RN role puts you right at the heart of hospital care coordination, ensuring smooth patient transitions through every step of their hospital stay. You'll work closely with physicians, social workers, and the nursing team to develop comprehensive care plans that prevent unnecessary readmissions and facilitate timely discharges.
