Job Description:

Join a local hospital as a Registered Nurse Case Manager focused on Emergency Department and medical inpatient coverage. This role offers the chance to be deeply involved in patient discharge planning, working closely with physicians, social workers, and multidisciplinary teams. You will manage a caseload typically assigned to a single unit, ensuring smooth transitions for patients as they move from hospital to home or another care setting. Expect to hit the ground running with approximately three days of orientation, and your workweek will be structured around five 8-hour shifts from 08:30 to 17:00, including every 4th weekend. This contract provides an engaging mix of independent patient management and collaborative team efforts, all in a busy hospital setting that emphasizes efficient patient flow and quality care.
