Job Description:

This adult nurse case management role at The Johns Hopkins Hospital offers the opportunity to coordinate patient care from triage through safe transfer to post-acute settings. You'll manage a dedicated caseload, working closely with interdisciplinary teams, payers, and patients' families to facilitate smooth transitions and ensure resource approval. The position involves utilizing EPIC and referral platforms like NaviHealth or AllScripts, with shifts scheduled from 8:30 am to 5:00 pm, Monday through Friday. No weekend, holiday, or on-call duties make this a predictable and manageable schedule. The work environment keeps you engaged in crucial coordination efforts while supporting patient safety and quality standards, all within a renowned academic medical center known for its high standards and collaborative spirit.
