Job Description:

This role calls for a detail-oriented Care Manager to support patients in an acute care hospital setting. You’ll be helping guide patients through their treatment journey by conducting comprehensive assessments, coordinating discharge plans, and collaborating with multidisciplinary teams to ensure smooth transitions of care. The position involves active participation in daily care management touchpoints, complex care meetings, and discharge planning, all aimed at optimizing patient outcomes and satisfaction. Expect to work with critically ill patients with diverse, complex needs, making this assignment both challenging and rewarding. You'll also be communicating with patients’ families, facilitating community resource referrals, and ensuring that all steps adhere to regulatory and departmental guidelines.
