Job Description:

Join a healthcare team as a Registered Nurse specializing in case management and discharge planning. In this 13-week contract, you'll coordinate patient transitions from hospital admission through post-discharge care, ensuring seamless communication among patients, families, payers, and vendors. You'll play a vital role in monitoring patient progress, resolving obstacles, and helping establish discharge plans—often within a lively inpatient setting that keeps you engaged. Expect to access and administer medications within your scope and according to State Law, all while working five days a week for 8 hours each day. This role offers a chance to make a tangible impact on patient outcomes during a critical phase of their care journey, all while experiencing a supportive team dynamic.
