Job Description:

Join a dedicated team as a Case Management Registered Nurse in Salt Lake City. This role involves coordinating the entire case management process, from patient admission to post-discharge follow-up, ensuring seamless care transitions across outpatient settings. You'll work closely with patients, families, payers, and internal teams to develop, monitor, and execute discharge plans, helping patients reach their health goals efficiently. The position offers a good dose of variety — some days you'll be reviewing patient progress, while others involve resolving obstacles or administering medications within your scope of practice. The schedule features four 10-hour shifts per week, allowing plenty of time to explore what Salt Lake City has to offer, while making a real impact in patients' lives.

Location: Salt Lake City, Utah
