Job Description

This travel contract is seeking an RN Care Coordinator (Case Manager) to manage an assigned caseload and coordinate care across the patient journey, from pre-admission through post-discharge. The RN Care Coordinator will assess clinical needs, apply care guidelines and payer regulations, monitor responses to therapy, and coordinate services prescribed by the interdisciplinary team. The role also includes mobilizing resources, supporting efficient care delivery, and contributing to quality, cost, and satisfaction outcomes.
