Job Description:

This 13-week travel healthcare assignment supports care coordination, referrals, outreach, enrollment, and individualized service planning in Sandwich, Massachusetts. You will partner with patients, families, service providers, medical directors, and third-party payers to coordinate needed services, update treatment plans based on patient response, prepare patients and families for discharge, and help establish practical care goals. The role also includes maintaining detailed records and supporting community-based healthcare plans across the surrounding area.
