Job Description:

Support patients and families throughout their healthcare journey as a Case Manager Registered Nurse focused on case management and discharge planning. You will monitor progress toward desired outcomes, identify and resolve barriers to care, coordinate with payers and vendors, and help develop, execute, and monitor individualized discharge plans from hospital entry through post-discharge services. This assignment offers a weekday schedule and welcomes first-time travelers with acute care case management experience. Medication access and administration may be required within your scope of practice and applicable state law.

Salt Lake City, Utah
Shift: 8-hour days, Monday through Friday, 08:00–16:30
Duration: 40 hours per week for 13 weeks
