Job Description:

Provide acute care case management and discharge planning support in an Apple Valley hospital setting. This assignment is designed for a seasoned Registered Nurse with at least two years of acute care case management or discharge planning experience and prior hospital experience. Work weekday day shifts from 8:00 a.m. to 4:30 p.m., with weekend coverage scheduled according to department needs, including approximately four to five weekend shifts within a six-week period. You will coordinate care transitions, support discharge planning, and help patients and care teams navigate the next steps in treatment. No floating is expected. Business attire or scrubs are appropriate.
