Job Description:

Join a respected hospital as a Case Manager RN for an 26-week travel contract in Portland, Oregon. This role involves floating between multiple units in a short-term acute care setting, managing inpatient care coordination, discharge planning, and ensuring patients receive smooth transitions from hospital to home. Expect to work eight-hour day shifts with a focus on supporting unit needs, including every other weekend. The ideal candidate will bring a minimum of three years of recent acute care hospital experience in care management, with the ability to adapt to a variable schedule and support staff during peak times like summer vacations and PTO. You will need a vehicle for this assignment, with various facilities in the Portland area and surrounding regions. This position offers an engaging opportunity to impact patient throughput and care coordination in a busy, fast-paced environment.
