Job Description:

Join us as a Case Manager Registered Nurse on a 12-week contract at a premier short-term acute care hospital. In this role, you'll be the go-to coordinator for patient discharge planning, assessment completion, and interdisciplinary collaboration. Your day will start with assessments for admitted patients, followed by participating in rounds and initiating referrals for services like home health, DME, and skilled nursing facilities. The unit covers a diverse range of medical-surgical cases, excluding psych and women and infants, offering a comprehensive experience in a hospital with over 1,200 beds. You’ll get to utilize EPIC and Allscripts charting systems, making your workflow efficient and up-to-date. This assignment provides a unique opportunity to be part of a Magnet-recognized, Level I Trauma Center environment, working with a team committed to excellence in patient throughput and coordination.
