Job Description:

Join us as a Case Manager Registered Nurse in the vibrant Washington, DC area, where you'll be at the forefront of coordinating complex care for diverse patient populations. This contract role involves engaging with high-risk diagnoses such as chronic conditions, behavioral health, and postpartum cases, making each day both challenging and rewarding. You'll make calls to enrollees, develop personalized care plans, and manage ongoing treatment within the EMR system, including guiding care through tools like Healthedge. The position offers a unique opportunity to experience the DC medical landscape while working in an environment that values your expertise in care coordination, discharge planning, and regulatory knowledge. Expect to work out of MedStar Georgetown starting May 1, with field visits twice weekly—Mileage reimbursement is provided.
