Job Description:

Join us for an engaging 13-week contract as an RN Care Manager in York, Maine, where you'll play a crucial role in coordinating patient care and discharge planning within a hospital setting. This position offers flexibility with variable shifts, including 8:30 am to 4:30 pm or four 10-hour days from 7:30 am to 5:00 pm, with no on-call requirements but weekend and holiday rotations. You'll liaise with healthcare teams, community organizations, and patients to ensure smooth transitions and appropriate resource allocation.
