Job Description:

Join us as a Patient Care Coordinator in Baltimore on a 9-week contract providing vital support within the Pediatric Complex Care Program. This role involves working with infants, children, and adolescents, helping to coordinate care and manage clinical activities. The position offers a hybrid schedule—up to three days onsite at Johns Hopkins Main Campus and two remote days—allowing for flexibility in your work routine. You'll be reviewing charts, managing the EPIC Inbasket, triaging calls, and assisting families with their care barriers, all while avoiding medication reconciliation or refills. This is an excellent opportunity for an RN with pediatric experience looking to make a meaningful impact through organized, hands-on care coordination.
