Job Description:

Join our team for a 13-week travel contract in Jackson, Mississippi, as a highly experienced Case Management professional. This role focuses on acute inpatient case management and discharge planning within a large Level 1 trauma medical center. Expect to handle a caseload of 15-20 complex patients daily, working on a rotating 36/48 schedule of 12-hour days. The position offers an energetic pace where quick learning and efficiency are essential, with minimal training provided. You'll collaborate closely with multidisciplinary teams, coordinate patient placements, and contribute to discharge strategies that influence patient outcomes.
