Job Description:

Work as a Case Management Registered Nurse in a fast-moving hospital-based setting in Atlanta, Georgia. In this travel contract, you’ll assess and evaluate discharge planning, coordinate services to the appropriate level of care, and ensure the treatment plan is initiated, reviewed, and re-evaluated daily. You’ll monitor day-to-day progress, identify barriers to discharge, and track length of stay to keep planning aligned with measurable outcomes.

The schedule is Monday through Friday with no weekends and no call. You’ll follow Medicare regulatory guidelines related to patient choice and required notice processes (including IMM notices and MOONS), supporting compliance throughout the discharge planning workflow. This assignment is well-suited for an experienced nurse who enjoys structured daily case progression and solutions-focused coordination—no first-time traveler onboarding; a minimum of 3 years of relevant experience is required.
