Job Description:

Join an acute hospital case management team as a Registered Nurse Case Manager in an outpatient clinic setting. In this role, you will focus on care coordination activities that support discharge planning, patient needs assessment, and arranging durable medical equipment (DME). You’ll work Monday through Friday on 8-hour day shifts (08:30–17:00), contributing to smooth transitions and well-organized next steps for patients and families.

This assignment also includes float coverage within the patient tower across MedSurg units as needed, keeping your days varied while staying focused on acute case management responsibilities. A key requirement for this position is demonstrated medical inpatient experience, along with the ability to manage coordination tasks in a regulated, HIPAA-compliant environment.

Weather:

Expect comfortable spring-like conditions with a mix of mild days and cooler evenings. Humidity can rise at times, and occasional light rain is possible, so having a light layer on hand is helpful.

Weekend/On-Call: No on-call requirement; weekend shifts may be picked up if desired and available
