Join the team at Johns Hopkins Hospital as an Adult Nurse Case Manager on the main hospital campus. This role involves coordinating care for adult patients in medicine and surgery units, ensuring smooth transitions from hospital to home or post-acute settings. You'll be actively involved in reviewing care plans, communicating with payors for approvals, and collaborating with multidisciplinary teams. The position offers an engaging mix of patient advocacy, clinical system management, and care coordination, all within a rewarding hospital environment.
