Job Description:

This RN Care Manager role is centered on conducting comprehensive initial assessments upon patient admission, using established screening tools to identify high-risk populations. The position involves coordinating multidisciplinary care efforts both within the hospital and during transitions to outside providers. You will be actively involved in discharge planning, patient and caregiver education, and arranging aftercare services, ensuring a smooth continuum of care. Expect to collaborate closely with healthcare teams, communicate crucial patient information, and participate in performance improvement initiatives.
