Job Description:

Join our team as a Case Manager Registered Nurse in a hospital setting where you'll navigate the complexities of inpatient care coordination. You'll work five days a week with 8-hour shifts from 08:00 to 16:30, including every 4th weekend and alternating holidays. Your primary focus will be coordinating patient discharges, evaluating medical necessity, and supporting the multidisciplinary team to ensure seamless patient flow.
