Job Description:

Join a vibrant team as a registered nurse serving as a Case Manager in an inpatient hospital setting. This role offers a Monday through Friday schedule from 08:30 to 17:00, with floating assignments across various units providing diverse patient cases and challenges. Weekend work is required once a month, with the flexibility to swap in a weekend day for a weekday shift as needed. The position involves coordinating patient care plans, facilitating communication among care teams, and ensuring smooth patient transitions. Epic EMR experience is essential, and candidates should bring at least two years of inpatient hospital experience, including first-time traveler applicants.
