Job Description:

Seeking a detail-oriented Case Management professional to join the team at Rapides Regional Medical Center in Alexandria, LA, for a 13-week contract. The role involves managing patient flow, conducting face-to-face interviews, and performing daily review of medical documentation using MediTech and Midas systems. This assignment offers a unique opportunity to collaborate across multiple hospital units, including Women & Children's and Eldercare, and to experience a fast-paced, interdisciplinary environment. The candidate will coordinate discharge planning, facilitate communication with patients and families, and ensure that care transitions smoothly. This position requires a proactive approach, excellent organizational skills, and the ability to adapt to various hospital settings within the facility.
