Job Description:

Join us as a Registered Nurse Case Manager in home health, where your expertise will guide patients through personalized care plans right in their homes. This role involves coordinating care with a dedicated interdisciplinary team, managing visit schedules to ensure timely and efficient service, and maintaining high standards of quality care. The position offers a unique chance to oversee patient progress directly, plan and organize care activities, and contribute to improving health outcomes in a home setting. It’s a fulfilling position for an RN who enjoys a mix of direct patient management and team collaboration in a setting that values attention to detail and compassionate oversight. Expect a schedule of five 8-hour days, working from 08:00 to 16:30, for a 13-week contract.
