Job Description:

Join a dedicated team as a Case Manager Registered Nurse in an outpatient setting, focusing on assessing, evaluating, and coordinating discharge plans to ensure smooth transitions for patients. This role involves daily evaluation of treatment plans, monitoring patient progress, and identifying barriers to discharge while following Medicare regulatory guidelines. The schedule consists of Monday through Friday, with daytime hours from 8:00 am to 4:30 pm, offering a predictable routine.
