Job Description:

This position is ideal for experienced Stroke Nurse Navigators looking to make a meaningful difference in patient recovery. You will serve as a central resource at a certified comprehensive stroke center, guiding patients and their families through the complex post-discharge process. Your days will involve providing in-depth stroke education, coordinating follow-up care, and collaborating with healthcare teams to identify and bridge gaps in care. The role combines clinical knowledge with community outreach, giving you the opportunity to influence both inpatient and outpatient outcomes while supporting community awareness initiatives. Expect a schedule of regular 8-hour daytime shifts on weekdays, with occasional participation in weekend community events to promote stroke prevention and awareness.
