Job Description:

Join a vital healthcare team as a Case Manager Registered Nurse, where your expertise will directly impact patient outcomes and healthcare efficiency. In this role, you'll coordinate utilization management, discharge planning, and post-acute care referrals, ensuring seamless transitions for patients across the care continuum. Your focus will be on managing acute patient populations, especially those presenting in the Emergency Department, with an emphasis on preventing readmissions and promoting patient wellness.
