Job Description:

Join us as a Case Manager Registered Nurse in beautiful Santa Rosa, California, for a 13-week contract that offers a rewarding opportunity to impact patient care from a unique perspective. In this role, you will coordinate utilization management, discharge planning, and post-acute care referrals, focusing on managing acute patient populations across the care continuum. This position involves assessing complex health needs, providing discharge coordination for high-risk patients, and supporting efforts to improve health outcomes while minimizing unnecessary hospital readmissions. The workday runs from 8:00 AM to 4:30 PM, with some weekend shifts required, giving you a balanced schedule to enjoy what the city and surrounding areas have to offer.
