Job Description:

Join us as a dedicated Case Manager Registered Nurse in Salt Lake City, Utah, for a 13-week contract that offers a meaningful chance to coordinate patient care from entry to post-discharge. This role involves managing case transitions, addressing patient and family needs, and collaborating with healthcare team members to develop and execute effective discharge plans. The shift is weekdays, from 8:00 AM to 4:30 PM, giving you a traditional schedule to enjoy your days outside of work. The setting emphasizes acute care, where your prior one-year experience in case management will shine. A focus on medication access and administration within legal scope is part of the role, all while supporting patients through their healthcare journey in the hospital or outpatient environments.
